Patient Information Leaflet: The FIB-4 Blood Test

Why Have I Been Asked to Have This Test?

You may have been invited for this test because you have risk factors for liver disease. Your doctor wants to:

  • Check for early signs of liver damage
  • Monitor your liver health over time
  • Decide if you need more tests, like a liver scan or ultrasound

These risk factors can include:

  • Obesity
  • Diabetes
  • Current or previous higher alcohol consumption
  • Previous diagnosis of fatty liver
  • Certain medications such as methotrexate

What Is the FIB-4 Test?

The FIB-4 (Fibrosis-4) blood test is a simple, non-invasive way to check how healthy your liver is. It helps estimate if there is any scarring (fibrosis) in your liver.

How Is the Test Done?

The FIB-4 test uses information from:

  • Your age
  • A blood test called AST
  • A blood test called ALT
  • Your platelet count

These results are used in a formula to give you a FIB-4 score.

What Do the Results Mean?

Your FIB-4 score falls into one of three categories:

Low (<1.3)Unlikely to have significant liver scarring
Intermediate (1.3–2.67)May need further tests (like a liver scan)
High (>2.67)Higher chance of liver scarring – more tests are usually recommended

Note: The cut-off values may differ slightly depending on your age.

What Happens Next?

  • Low score? Great! Your liver is likely in good shape. Your doctor may still check your liver regularly.
  • Intermediate or high score? You may need further tests, such as a FibroScan, we will be in contact with you to book in you for the scan.

Is the FIB-4 Test a Diagnosis?

No. The FIB-4 test is a screening tool. It can’t confirm liver disease on its own but helps your doctor decide what to do next.


How Can I Keep My Liver Healthy?

✅ Eat a healthy, balanced diet
✅ Stay a healthy weight
✅ Be active and exercise regularly
✅ Take any medicines as your doctor recommends
